What You Need When Your Insurance Requests a Referral for Medical Equipment
If your health insurance company asks for a referral or prior authorization before covering medical equipment, don’t worry — this is a common step, and we’re here to help walk you through it.
Here’s what’s typically required:
1. A Prescription from Your Doctor
Insurance companies need proof that the equipment is medically necessary. Your doctor or healthcare provider must write a prescription that includes:
The type of equipment needed (e.g., CGM, wheelchair, PureWick™ system)
The medical reason for the equipment (diagnosis or condition)
How often it will be used or needed
Duration of use (temporary vs. ongoing)
2. A Medical Referral (If Required by Your Plan)
Some insurance plans require a formal referral from your primary care physician (PCP) or specialist. This is usually a short form or note sent from your provider’s office directly to the insurance company or to us.
We recommend contacting your provider’s office and asking:
“Can you send a referral for my medical equipment to my DME provider?”
The office staff will send the necessary forms to us. You can also give us the doctor's name and contact information, and we can reach out to them to request it for you.
3. Clinical Notes or Medical Records
In some cases, insurance will ask for supporting medical documentation. This may include:
Recent visit notes
Diagnostic test results (e.g., for diabetes or mobility assessments)
History of the condition or treatments tried previously

4. Insurance Information
Make sure we have your most up-to-date insurance information. Send us an email at orders@metromedsupply.com with the following:
Insurance card and Identification card
Policyholder information
Primary care provider and referring doctor (if applicable) contact information
This helps us submit the correct paperwork the first time and avoid delays.
